HealthPort Pennsylvania medical records fee settlement
If you paid HealthPort for records from a Pennsylvania healthcare provider for a life or health insurance application, you may have been charged more than Pennsylvania law allows.
Eligibility and administrator approval apply. Payment is not guaranteed.
Could this apply to you?
Paid HealthPort for Pennsylvania medical records · 2009-2026
- Did you request medical records from a Pennsylvania healthcare provider for a life or health insurance application?
- Did you pay HealthPort (now Datavant) for copies of those records?
- Did you make the request and payment between December 17, 2009 and July 23, 2026?
- Were you charged HealthPort's fees rather than a rate you had agreed on in advance?
- Do you have the mailed class notice listing your Class Member ID (or will you contact the administrator to get one)?
What you’ll need
Filing online works only with the Class Member ID (Notice ID + PIN) from the mailed class notice, which opens your pre-filled Proof of Claim. If you did not get a notice, you cannot file directly and must first contact the settlement administrator.
Your next step
We check whether you have the mailed notice with a Class Member ID, which is needed to file; without one you must contact the settlement administrator first.
