Ordered from one of four SF Domino's stores · 2023-2026
- Are you a California resident who bought from a Domino's in San Francisco?
- Did you buy from the Domino's at 3116 Noriega St., 320 Bayshore Blvd., 5200 Geary Blvd., or 728 Geary St.?
- Did you pay a 6% Health Care Security Ordinance (HSCO) charge on that order between December 4, 2023 and August 12, 2026?
- If your orders weren't linked to an email you gave the store, can you send proof of purchase showing the amount paid?
What you’ll need
Keep receipts or order records that show the 6% HSCO charge and the amount paid at one of the four San Francisco stores. If your orders were not linked to your email, you must first email or mail proof of purchase to the administrator, who will send you a login to file the cash claim.
Your next step
We check whether your order was at one of the four stores and included the 6% HSCO charge during the class period.
