• Do you have the LoginID and PIN from your postcard notice, or can you request them from the administrator?
• Can you submit your claim form by December 24, 2026?

What you’ll need

Find the LoginID and PIN printed on your mailed postcard notice; if you don't have them, ask the settlement administrator for them. If you claim out-of-pocket losses, keep receipts or statements that weren't made by you, showing costs from 2024 onward that are linked to the breach.
